Composes the instructions for a running server.

## Parameters

- `policy` (`ServerPolicy`): What this server will do.
- `callerPaneId` (`string?`): The pane this server runs in, when it runs in one.

## Returns

The instructions.

## Raises

- [`InvalidOperationException`](https://learn.microsoft.com/dotnet/api/system.invalidoperationexception): The fixed guidance no longer fits the budget. That is a bug in this file
rather than a condition to degrade around: silently dropping a segment
would leave a model without the rule it was dropped from.
