Planner groups a plan's operations into tmux invocations.

Dispatches must contain every operation exactly once and in recorded order.
[Plan.Run] validates this before reaching tmux.

Operations with [Op.Chainable] false must run alone because command-list
stdout cannot be attributed to an individual operation.
